You're a professional making a positive difference in our community, through health and wellness practices, education and more. We want to thank you for all you do and invite you to be part of a special program.

The Rainbow Blossom Wellness Ambassador Program provides members with exclusive pricing, services and benefits, including a 10% discount off your purchases at Rainbow Blossom Natural Food Markets when shopping at any of our five retail markets, anytime.

In addition, Rainbow Blossom will frequently have samples and gift baskets… just for you! As an exclusive member of this program, we would greatly appreciate your referral for your clients to shop with us for the products you recommend to them.

Professionals such as health coaches, chiropractors, doctors and naturopaths, acupuncturists, massage therapists, fitness instructors, healthy chefs, birth doulas, lactation consultants, etc. are invited to apply. Please complete the application and fax in at least two forms of professional identification from the list below.

Terms and Conditions: These Terms & Conditions set forth the terms and conditions for membership in the Rainbow Blossom Wellness Ambassador Program (the "Program"). Rainbow Blossom may change the Rainbow Blossom Wellness Ambassador Program or these Terms & Conditions from time to time. Rainbow Blossom will post such changes online at www.RainbowBlossom.com/wellness-ambassador-program. Any changed Terms & Conditions become effective immediately upon being posted, and your continued use of membership constitutes your agreement to the changed Terms & Conditions.

How to Qualify: The Rainbow Blossom Wellness Ambassador program membership is only granted to local professionals such as health coaches, chiropractors, doctors and naturopaths, acupuncturists, massage therapists, fitness instructors, healthy chefs, birth doulas, lactation consultants, and the like. You must reapply on an annual basis to qualify.

The Program is designed for Louisville Metro and Southern Indiana members only. We reserve the right to make all membership determinations at our sole discretion. Each wellness professional must individually apply for membership by submitting their individual qualifications. Incomplete applications will not be accepted. Employees of Rainbow Blossom, Inc. are not eligible for membership.

Membership: Membership is not transferable. A membership coupon code can only be used by the named member. The member must present his/her valid email coupon and valid photo identification in person to receive the trade discount at time of purchase. Misuse of membership may result in membership termination. Misuse includes, without limitation: any resale of merchandise purchased using the discount through any retail outlet (e.g., resale on or through any retail or auction Web site or store), reimbursement of discount upon merchandise return, transfer of membership, allowing others to use the membership discount and submitting false, expired or misleading information in connection with a membership application. You must use your Rainbow Blossom Rainbow Rewards scan card for making purchases in order to receive the 10% discount. You must reapply to the program on an annual basis to qualify.

Discount: A 10% discount is valid at Rainbow Blossom Natural Food Markets only and excludes sale, clearance items, and ‘everyday low price’ items. Any discount through the Program is not valid with any other offer, purchase of gift cards or promotional items or taxes. The discount is not redeemable for cash. Rainbow Blossom reserves the right to change the list of items not eligible for the discount at its discretion. Any such changes will be posted as changed Terms & Conditions, discussed above. Termination: Rainbow Blossom reserves the right to terminate a membership or cancel the Rainbow Blossom Wellness Ambassador Program at its sole discretion, with or without notice to the member. 1. Certification indicating profession within the wellness industry, OR

2. Certificate of completion from an accredited wellness program.

AND one of the following:

1. Business card indicating profession within the wellness industry.

2. Letter printed on letterhead, certifying role as a wellness professional.
